Eligibility

If you are a full-time employee of Furmano's regularly working over thirty (30) hours per week, you are eligible to participate in the Healthcare and Benefits Program on your 90th day of employment. You have the option to enroll yourself and your eligible dependents in Furmano's benefits program.

If you don’t enroll by the date when your benefits will go into effect, you will not have the opportunity to do so until
the next open enrollment period, unless you have a Qualifying Life Event.​

Eligible dependents include:

• Your legal spouse (if your spouse is employed and offered medical coverage by their employer, they are not eligible for coverage under the Furmano's plan)

• Your children up to age 26. Only those dependents who meet the eligibility requirements can enroll for coverage.
